Angiotensin Converting Enzymes Inhibitors (ACEi) and Angiotensin Receptor Blockers (ARB) are the drugs of choice in managing hypertensive conditions in patients with co-morbidities such as Diabetes or Chronic Kidney Disease. They are especially helpful in patients with concomitant kidney diseases because they decrease the pressure exerted on the kidney by the blood, thus decreasing stress on the kidney, while also decreasing blood pressure. Like all medications, there are side effects and the most common for ACEis are cough, ARBs usually do not produce this. These drugs should not be given to pregnant patients; they can cause oligohydramnios, renal dysplasia, intrauterine growth retardation and many more complications.In addition to their use in blood pressure reduction these medications have also been shown to reduce mortality in patients who have experienced a myocardial infarction and those with chronic heart failure.

Adrenoreceptor Blockers is a group of medications that are used in managing hypertension in patients. They act on adrenergic receptors such as beta-adrenergic or alpha-adrenergic receptors. The medications can lower the heart rate, the heart contractility, and vasomotor tone of the blood vessels. The main side effects of the beta-adrenergic blockers are fatigue, depression, or even serious hyperkalemia. And of course, the contraindications to beta-adrenergic blockers are conditions that associate with the depressed function of the heart such as Decompensated CHF or symptomatic bradycardia. It is also important to know the clearance mechanism of each individual beta-adrenergic blockers. Certain beta-adrenergic blockers are cleared via the Kidney, whereas others are cleared by the Liver. The Alpha-adrenergic blockers are too help with treating hypertension. However, alpha-adrenergic blockers’ actions are at the blood vessels, not the cardiac tissues.  The side effects of the alpha-adrenergic blockers are more profound than the beta-adrenergic blockers’. One important side effect one should pay attention to this group is the rebound hypertension or hypertensive crisis.

Diuretic medications are useful in treatment of hypertension by reducing the kidney’s reabsorption of the euvolemic water. There are 6 classes of Diuretics that act on different receptors along the kidney’s tubular system. Loop diuretic group is the most potent diuretic medications. However, physicians should take into considerations of the side effects of these diuretics. Most common side effects among these diuretics are the depletion of the body’s important chemical substances such as Sodium, Potassium, Chloride, Magnesium,etc. As chronic hypertensive patient will develop chronic kidney failure, diuretic medications may become less efficacious. Most of these diuretics will require optimum renal function.
 

Platelets are primary hemostasis via activating vWF, adhesive to collagen, releasing chemical messenger for the thrombotic process, a platelet plug function. However, there are 6 types of triggers of the platelet aggregations, and the antiplatelet therapy is geared toward these triggers. Medication therapies include Aspirin,  ADP antagonists, PDE inhibitors, Glycoprotein IIB/IIIA receptor antagonists. Aspirin will permanently bind to the COX activated site; this will prevent the production to TXA2. Aspirin will render the platelet permanently inactive. Other medications therapies are also effective by inhibiting the platelet’s chemical signals.
